How to Make a Coaching Appointment
1. Call to Schedule
If you would like to come in for counseling, call the office first at 770-502-8050 and set up an appointment. If you reach our voice-mail, leave a message telling us what you need and leave your number. We will call you back in a timely manner.
2. Fill Out Intake Forms
After scheduling an appointment, download the intake form below.* Make sure that you completely fill out the intake form and sign where necessary. If you have any problems with downloading or you do not have a printer, call us at 770-502-8050 and we will work out another way to get you the forms you need.
*In order to view the intake form you will need the free Acrobat plug in from Adobe which can be downloaded here.
3. Bring Intake Forms
Bring the signed intakes with you to your first coaching appointment. Make sure that you arrive 10 minutes early to your first appointment to make sure everything is in order.
